![]() ![]() Only studies that had compared stapled and hand-sewn anastomosis, with or without bowel resection, were included in the analysis. All full articles were obtained for the included studies. Two authors independently assessed the titles and abstracts of the identified studies. The reference lists in the identified studies were also searched in order to identify further studies. The databases used for the literature search included MEDLINE, EMBASE, and Cochrane Controlled Trials Register. Systematic search strategies was used to identify the relevant studies and electronic search was performed. The primary endpoint was anastomotic leak and the secondary endpoint included bowel obstruction, wound infection, bleeding, operating time, time to first defecation, length of hospital stay, re-admission, re-operation and 30 days operative mortality. There was only one study with more than 1,000 participants. One study was a prospective non-randomised and one was a retrospective analysis of a prospective database. There were only 4 randomised control trials in the literature. All relevant studies were considered, irrespective of language and publication status, for this review. MethodsĪll published randomised controlled trials and quasi-randomised controlled trials comparing stapled and hand-sewn closure of loop ileostomy were eligible for inclusion. This review aims to compare stapled versus hand-sewn anastomoses with regard to all documented parameters of perioperative morbidity in addition to looking at the effects of small-bowel resection on anastomotic leak and bowel obstruction. A systematic review, with a pooled analysis of results, might provide a more definitive answer.Īlthough systematic reviews on this topic already exist in the literature, none of them has looked into the effect of small-bowel resection during the ileostomy reversal procedure. Individual trials comparing hand-sewn with stapled closure of loop ileostomy showed different outcomes, perhaps because they lacked statistical power. The side-to-side stapler anastomosis for closure of ileostomy was introduced into practice in the 1980s. The traditional method for reversing a loop ileostomy involved a hand-sewn end-to-end bowel anastomosis. Once the anastomosis has healed or a fistula successfully repaired, the loop ileostomy may be closed. It can also be used as a diversion in severe fistulating perianal Crohn’s disease. The most common conditions where a loop ileostomy may be used include resections for rectal cancer, ulcerative colitis, diverticular disease and familial adenomatous polyposis. This technique is primarily used to facilitate the healing of a low colorectal anastomosis to diminish the consequences of an anastomotic leak. Performance of bowel resection does not significantly increase the incidence of anastomotic leak or small-bowel obstruction.Ī defunctioning loop ileostomy is a commonly used surgical procedure to allow faecal diversion from a distal anastomosis. The rate of small-bowel obstruction was higher in the hand-sewn group. Conclusions: There was no significant difference in the rate of anastomotic leakage between the hand-sewn and stapled techniques. There was no difference in the incidence of anastomotic leak and small-bowel obstruction in the hand-sewn anastomosis between patients with or without bowel resection. Results: Of the 4,917 patients in 15 identified studies, 3,406 had hand-sewn and 1,511 stapled anastomosis. All randomised clinical trials, prospective and retrospective studies comparing hand-sewn with stapled closure of loop ileostomy were included. Methodology: Relevant studies were identified from MEDLINE, EMBASE and the Cochrane database. This review aimed to compare hand-sewn with stapled anastomotic technique for closure of a loop ileostomy and looked at the effect of bowel resection on the complication rates. A systematic review, with a pooled analysis of results, might provide a more definitive answer. Background: Individual trials comparing hand-sewn with stapled closure of loop ileostomy show different outcomes due to lack of statistical power. ![]()

The band's 2000 debut, Mer de Noms, was a surprise hit, while the ensuing tour was a sold-out success as well. Even with their post-punk influences, they executed their music with the aesthetic of prog rock, alternating between long, detailed instrumental interludes and lyrical rants in their songs. You can turn your IoT product into a Wi-Fi access point with a call to the static accessPoint function of the WiFi class. The next sections summarize how to use these features in your own projects. I built a few clocks for remote friends and family, and I didn’t want them to have to change any code or use the command line to change the Wi-Fi settings. That is, to make it easy for the user to connect their clock to their home network without using a touch screen. The first challenge of the clock project was to simplify Wi-Fi configuration for the user. I had a few extra NeoPixels strands left over from my Christmas lights project, and I had an idea to use them to make a classic seven-segment display clock to give as a gift. The example code in this post comes from this project. I recently used these tools to make an ESP32-based clock. However, there are some devices that don’t need a screen. An IoT product that incorporates a well designed touchscreen interface is a joy to use. That might surprise you because a key feature of the Moddable SDK is the ability to add interactive, animated touch screens to IoT products. Many of these outcomes, both good and bad, will radically alter the payoffs of various life decisions you might make now. You can and should decide what you think that probability is, and conditional on that happening, your probability of various outcomes. There is some probability that humanity will create transformational AI soon, for various definitions of soon. I am going to step up here, and dare to answer these, those added by Twitter and some raised recently in personal conversations.īefore I begin, it must be said: NONE OF THIS IS INVESTMENT ADVICE. While some ignore the issue, others, in various ways, dramatically overreact. Remember that you are the easiest person to fool. It does mean that you should consider your actions carefully when doing so, and the possibility that you are fooling yourself. That does not mean that you should not make any attempt to improve our chances. ![]() Remember that the default outcome of those working in AI in order to help is to end up working primarily on capabilities, and making the situation worse. In exchange, one avoids major negative outcomes that potentially include things like missing out on the important things in life, ruining one’s financial future and bouts of existential despair.Īlso the risk of doing ill-advised counterproductive things in the name of helping with the problem. It leaves value on the table, both personally and for the world. They live their lives as if such risks do not exist. Many effectively suppress this info, cutting the new information about the future off from the rest of their brain. People who do react by worrying about such AI outcomes are rarely reacting about right given their beliefs. Reacting Properly To Such Information is Hard If this is something that is going to impact your major life decisions, or keep you up at night, you need to develop your own understanding and model, and decide for yourself what you predict. That does not mean you should adapt my position, or anyone else’s position, or mostly use social cognition from those around you, on such questions, no matter what those methods would tell you. Thus I believe this is very likely, although there are some (such as Eliezer Yudkowsky) who consider it more likely still. There are what I consider very good arguments that this is what happens unless we solve extremely difficult problems to prevent it, and that we are unlikely to solve those problems in time. There is also the highly disputed question of how likely it is that if we did create an AGI reasonably soon, it would wipe out all value in the universe. These are definitely world types my model says you should consider plausible. There are also good reasons that AGI, or otherwise transformational AI, might not come to pass for a long time.Īs I say in the Q&A section later, I do not consider imminent transformational AI inevitable in our lifetimes: Some combination of ‘we run out of training data and ways to improve the systems, and AI systems max out at not that much more powerful than current ones’ and ‘turns out there are regulatory and other barriers that prevent AI from impacting that much of life or the economy that much’ could mean that things during our lifetimes turn out to be not that strange. This includes good reasons to worry about AI wiping out all value in the universe, or AI killing everyone, or other similar very bad outcomes. There are good reasons to worry about AI. Some people, including some of those same people, need practical advice. Some people ( although very far from all people) are worried that AI will wipe out all value in the universe. ![]() |
